Image
guided surgery allows accurate computerized localization in three dimensions
within the brain using a frameless system.
The
Radionics
OmniSight Optical Tracking System used at MetroWest Medical Center
permits accurate frameless CT or MR guidance which is accurate to <
5 mm.
The
OmniSight
OTS
works by registering the patient's head with scalp fiducials relative
to the images and locating the position of the surgical instruments in
this space by light reflected off spheres embedded on them detected by
an optical camera and transmitted to the computer for analysis.
Tumor
size and depth can be visualized before making an incision and during the
resection to aid with decisions regarding the extent of resection.
Mini-craniotomy
with a small incision and bone flap can be performed as a direct consequence
of this accurate localization.
Planning
the optimal trajectory to the tumor can be optimized to provide the shortest
route or avoid critical areas of eloquent brain to prevent postoperative
neurological deficits.
